UC College of Medicine Space Capacity Exception Request

During the fall and spring semester (subject to further guidance) the maximum capacity for space utilization in the UC College of Medicine is 10 people per space. To have > than 10 people per gathering in an allowable space (UC P+D+C has identified a higher capacity for a given space) requires an exception approved by the Dean’s office. Please complete this form to request an exception to the maximum capacity of spaces with capacity > than 10 people. Requests for gatherings of more than 50 people will not be considered. Note: requests must be submitted 10 business days in advance of the event. For any event or session in a UC COM space participants must follow the expected social distancing guidelines.

Brief Description of the event/course.

Provide the rationale for your request; why an exception is needed and address any added precautions being taken to ensure individuals safety.

Describe the individuals attending the event/course and include total number of students including instructor/course director.
