Using Bearcats Health App: Tip Sheet

Below are helpful instructions and tips on how to use the Bearcats Health App and fill out the required University of Cincinnati Standardized Immunization Form.

• All students are required to use the University of Cincinnati Standardized Immunization Forms in the Bearcats Health App. No other documents will be accepted. DO NOT attach outside documents. Your form will be rejected by the system if there are outside documents attached.
  • General Population/International Student Immunization Form (PDF)
  • Health Professions Student Immunization Form (PDF)

Previous Med+Proctor Patients

• Your compliance status from Med+Proctor was transferred to the Bearcats Health App as of 9/18/23, when Med+Proctor became unavailable. If you became non-compliant as of 9/18/23, you will be required to submit a fully completed immunization form to the Bearcats Health App to become compliant. You can only use the University of Cincinnati Standardized Immunization form. No other forms will be accepted.
• Unfortunately, your previously uploaded documents will not transfer from Med+Proctor to the Bearcats Health App, only your compliance status. We apologize for the inconvenience.
• If you had a previous exemption in Med+Proctor for any vaccine, you will need to re-submit through the Bearcats Health App exemption process.
• If you had immunization documentation in Med+Proctor that you want to access, you will have to pay the $10 fee for Med+Proctor Pro in order to download your records and use them whenever you need them. However, your documents from Med+Proctor will not be accepted in the Bearcats Health App, as the Standardized Immunization Form must be used, so you will still have take your documents to a medical provider to have them review, fill out your form, and sign it.

Tips

Follow these tips when filling out the Standardized Immunization Form so that the Bearcats Health App system can properly read it:

• Be sure dates are written within the designated boxes, not outside of them
• Make sure dates are written as MM/DD/YYYY
• Make sure your medical provider signature, credentials, office stamp fall within the designated boxes, not outside of them
• When you take a picture of your form, make sure it is square in the frame
• Signature validation requirements for all Standardized Immunization Forms include one of two options:
  • Medical provider's office stamp alone, or in conjunction with any of the other requested information
  • If no office stamp, the validation must include ALL of the below:
    • Signature AND printed name
    • phone OR address
    • Provider credentials OR professional number
• PDF and image files are able to be used. Those are the only two file types that will be accepted. You can either scan your form or take a photo of your form with your phone. If you have image files, the Bearcats Health App system will prompt you to merge them and it will convert them to a PDF file for you.
• If you are in process with a vaccine series, you are able to submit partial forms with the information for each new dose as you move through the series, but you must include all previous doses of the series each time you submit a form.
  • For example, if you are in process with a Hepatitis B vaccine, which is a 3-dose series, you should include your first dose on your initial form. You will need to wait four weeks to have your second dose. At that time, you may upload a form that just includes dates for your Hepatitis B vaccine, but you must include the date of your first dose as well as the date of your second dose in order for the Bearcats Health App system to read your form. You will then have to wait five months from your second dose to receive your third. At that time, you may upload a form that only includes dates for this Hepatitis B series, but you must include the date of your first and second doses along with your third/final dose.

Health Professions Student Specific Requirements

All Health Science students are required to complete an annual TB screening questionnaire and receive a flu vaccine no later than 11/01/2025. You will be marked as non-compliant until these are completed in the Bearcats Health App.

• Annual TB Screening: You will complete the TB screening in the Bearcats Health App.
• Flu Documentation: Once on the Welcome page in the Bearcats Health App, click the "My Immunization Status" tab at the top left of the screen, which should reveal a list of immunizations. On the left, scroll down to Influenza and click on the button provided. Please answer the populated questions to provide details about your vaccine. Afterwards, please use the "Upload" button to submit your original record to the portal. If you submitted your flu vaccine into Med+Proctor, you must upload it again into the new system to be compliant.
